  • Action: Funeral director intern license revoked for 10 years, and fined $10,000.

Tacoma — Fleig Threlkeld Funeral and Cremation Services

  • Finding: Unprofessional conduct, and using false, deceptive, or misleading advertising.
  • Action: Fined $1,500.

Tacoma — Patrick Threlkeld

  • Finding: Unprofessional conduct, and using false, deceptive, or misleading advertising.
  • Action: Reprimanded.

Tacoma — Robert Liedle

  • Finding: Unprofessional conduct, and using false, deceptive, or misleading advertising.
  • Action: Reprimanded.

February 2008

Kent — Cady Cremation Service

  • Finding:

    • Negligence.
    • Performing a cremation without the required permits.
  • Action: Licenses of Lisa Anasazi and Steve Cady suspended (suspension stayed) and Cady Cremation Service fined $2,000.

Vashon — Island Funeral Service

  • Finding: Unprofessional conduct through negligence, and failure to follow the directions of the decedent’s next of kin relative to the right to control disposition.
  • Action: Dennis Petersen and Island Funeral Service reprimanded, and Island Funeral Service fined $2,000.

April 2007

Everett — Evergreen Funeral Home and Cemetery

  • Finding: Unprofessional conduct, and failure to follow the directions of the decedent’s next of kin relative to the right to control disposition.
  • Action: Evergreen Funeral Home and Cemetery and Funeral Director Judy Jewell reprimanded, and business fined $5,000.

Kent — First Call Plus of Washington

  • Finding: Misrepresentation, and aiding and abetting unlicensed practice.
  • Action:
    • First Call Plus of Washington, Jerry Webster, Gloria Webster, Jason Hardt, Brian Noonan, Joe Tommasi, and R.W. Fisher reprimanded.
    • Business fined $4,000.
    • Jerry Webster fined $2,000.
